- Community Type II diabetes - this service includes training for clinicians to manage patients within primary care and an intermediate service involving primary and secondary care for less controlled patients.
- Community ultrasound - Regular sessions run from local practices.
- Screening for Atrial Fibrillation - Being undertaken by our practices.
- Screening for undiagnosed Type II diabetes.
- Screening for COPD - ensuring patients are diagnosed early and appropriate treatment put in place.
- Intermediate Cardiology Clinic - this is an innovative service that will allow patients not needing to go to IpswichHospital to be seen more locally. The clinic will take around a third of current referrals and be staffed by a local consultant and GP Clinical Assistants.
- Intermediate Urology Clinic - as with cardiology this clinic will take around a third of current referrals and be staffed by a local consultant and GP Clinical Assistants.
- Early identification of patients at risk of an avoidable hospital admission - We will be monitoring various sources and ensuring patients receive the services they need and at an appropriate time.
